Commercial CAR-T in China: Pricing Breakdown & Sources

This page explains how commercial CAR-T drug pricing in China is established from official NMPA-approved list prices, how standard patient discounts apply to international patients, what hospital bills beyond the cell product typically look like at public wards, public international departments, and private international hospitals, and how to identify fraudulent low-cost offers.

1. The Official ¥1,150,000 List Price & ¥300,000 Reduction

In China, commercially approved autologous BCMA CAR-T therapies—福可苏® (eque-cel, NMPA approval 国药准字S20230006) and 赛恺泽® (zevor-cel, NMPA approval 国药准字S20240007)—are registered on official provincial public pharmaceutical procurement platforms with a regulated nationwide retail list price of ¥1,150,000 RMB for cell manufacturing and batch release. This is a regulated drug list price, not an all-inclusive hospital package.

Commercial manufacturers provide an official ¥300,000 RMB standard patient assistance reduction program. International patients are equally eligible for this reduction across all authorized Chinese hospitals (both public tertiary centers and private international hospitals), bringing the net cell product cost to approximately ¥850,000 RMB before hospital admission, tests, and procedural care.

Pricing Integrity & Scam Prevention

To protect international patients from fraudulent broker packages or unregulated investigational products, keep these two commercial benchmarks in mind:

Drug Cost Rule: Licensed NMPA commercial cell drug alone costs ~¥850,000 RMB (~$120,000 USD). Any offer quoting drug costs below $120,000 USD requires extreme caution.

All-Inclusive Total Rule: Genuine private international care (drug + inpatient hospital stay) typically totals $160,000–$215,000 USD. Any broker offering an “all-inclusive private package” below $160,000 USD is a high-risk warning sign.

2. Hospital Costs Beyond the Cell Product — Three Tiers

The cell product is billed separately from the hospital. A typical commercial myeloma CAR-T episode still includes work-up (PET-CT, marrow, echo, infection panel), apheresis, Flu/Cy lymphodepletion, 7–14 days in a HEPA isolation room, daily labs, supportive medicines, and optional bridging or ICU days. The 8–10 week stay in China is mostly outpatient lodging during manufacturing and the 30-day safety window — that lodging is not in the hospital bill.

No Chinese hospital publishes a binding, itemized CAR-T non-drug package online for international patients. The ranges below are order-of-magnitude estimates built from posted municipal tariffs × a typical isolation stay, plus work-up and supportive care. They are not quotes. Ask the treating hospital for a written estimate after record review.

How the public-ward range was built

Posted public tariffs for ordinary/laminar beds are low: Shanghai Xinhua lists laminar-clean beds at ¥90/day and ordinary single rooms from ¥398/day; three-bed wards are ¥68/day. Isolation plus Flu/Cy, PET-CT, marrow, echo, labs, and supportive medicines still add up. Public compilations of China CAR-T episodes typically put hospitalization around ¥20,000–50,000, tests ¥10,000–30,000, lymphodepletion ¥10,000–30,000, and supportive care ¥10,000–50,000. Adding those blocks, and allowing for one tocilizumab course or a few extra isolation days, gives about ¥80,000220,000 before catastrophic ICU. For comparison, U.S. peri-infusion hospital care excluding the cell product is about $23,000 (Jagannath et al., 2023).

How the international-department range was built

Ruijin Hospital’s published international-department rules are the cleanest official template: tests, labs, and drugs stay at ordinary public prices; only bed, inpatient physician visit, and nursing use special-needs prices. Shanghai Children’s Hospital’s June 2026 posting lists special-needs single rooms at ¥1,200/day, internal-medicine visit ¥1,000/day, and level-1 nursing ¥700/day — about ¥2,900/day for those three lines alone. Sun Yat-sen First Affiliated posts special-needs beds at ¥1,200–2,800/day. A 2015 Shanghai health-policy paper, citing the 2012 municipal special-needs rule (沪卫规财〔2012〕022号), recorded special-needs beds up to ¥6,000/day and a 2013 mean special-needs bed-day cost of ¥4,662. Twelve to sixteen isolation/monitoring days at ¥2,900–5,000, plus public-rate work-up and chemo, lands near ¥150,000350,000.

How the private-international range was built

Licensed private international hospitals in China typically do not publish fixed, itemized all-inclusive CAR-T packages online. Public clinical explainers split CAR-T bills into two distinct components: the regulated commercial cell drug and non-drug hospital care. Private single-room oncology isolation stays in Tier-1 cities incur higher daily room, nursing, and private test tariffs. Independent industry compilations place a complete China commercial CAR-T episode at approximately ¥1.10–1.65 million RMB. Subtracting the ~¥850,000 discounted drug leaves an indicative hospital-side non-drug band of about ¥250,000600,000 for an uncomplicated to moderately complicated case.

Not in these ranges: international flights, 8–10 weeks of apartment lodging, meals, interpreters, or a long ICU stay. Severe CRS (Grade ≥3) is uncommon with China’s approved products (~1.0%–6.9% in registrational cohorts) but can add tens of thousands of RMB if ICU days accumulate. Bridging chemotherapy, if needed during manufacturing, is also extra.

3. Inpatient Care, Bridging, and ICU Monitoring

Severe cytokine release syndrome (Grade ≥3 CRS) occurs in only ~1.0% to ~6.9% of myeloma patients treated with China’s approved commercial products. Among the minority of patients who require temporary intensive care escalation, published clinical cohorts report a median ICU stay of 4–5 days (Constantinescu et al., 2023):

Grade 1–2 Mild CRS (Standard Ward Care)

~90% of treated patients

Managed on the routine hematology ward with antipyretics, supportive fluids, and IL-6 receptor antagonists (such as tocilizumab) as clinically indicated.

ICU Escalation (Severe CRS / Neurotoxicity)

Occurs in a small minority; median 4–5 ICU days

In China, all CAR-T administering hematology centers must maintain an on-site ICU. Intensive care stays are billed at standard hospital critical care daily tariffs based on actual clinical interventions.

4. How to read a quote against the commercial floor

A third-party “all-in CAR-T in China” price should be checked against the drug floor and the hospital tier. The numbers below are planning ranges, not a quote:

1. The ~$120k–$126k Drug-Only Floor

The net NMPA commercial cell product alone is ~¥850,000 RMB (~$120,000–$126,000 USD) before any hospital bills. Any broker offer claiming an all-inclusive trip under this threshold sits below pure cell manufacturing cost and cannot cover licensed commercial cell manufacturing.

Rule: Below ~$120k rarely covers licensed commercial CAR-T

2. Public General Ward (~$135k–$158k)

Legitimate public general ward episodes land around ~$135,000–$158,000 USD total. However, public hospitals bill directly in RMB with no intermediary packages; ward rounds are in Mandarin and foreign families must arrange their own independent daily logistics.

Rule: Real public care = Direct RMB hospital billing

3. Private-hospital packages under $150k

Legitimate private international hospitals with English care cost ~$160,000–$215,000 USD total. Third-party brokers advertising “full private concierge packages with lodging and translation for under $150,000 USD” often will not match a licensed private-hospital invoice, or describe a pathway that is not commercially released.
